Okay, I'm starting to think it's something to do with having multiple reasearch grants in play at once, or maybe even more than one in play when she gets incapacitated...
I only played Research Grant once; I discarded the other to an environment effect. My one Research Grant was in play when she was incapacitated.
Well, one success claims to have kept pushing the limits in play, so if sonic damage is a factor, she can at least deal it to herself.
Here’s another fail, but it’s with sonic damage and no reshuffles. Just in case we’re on the wrong track entirely.
Ok, went through the 2 success logs I have here and in both instances the following was true:
1. Tachyon played at least 10 cards in a single turn.
2. Tachyon did no Sonic damage to Progeny.
3. Tachyon reshuffled her deck at least twice.
4. Tachyon was incapcitated
5. The Vanilla Freedom Five was used
6. Both were in Megaloplis (which I think we've all figured out is going to be a requirement for all the FV unlocks).
7. Both were victorys (which I think we've all figured out is going to be a requirement for all the FV unlocks).
So here are the conditions I'm assuming right now:
1. Meet the pre-req games.
2. Win against Progeny in Megaloplis with the Freedom Five (Standard)
3. Tachyon must be incapactitated
4. Tachyon can not deal sonic damage to Progeny
5. Tachyon must reshuffle her trash at least twice
6. Tachyon must play at least 10 cards in a single turn.
Things to test:
1. Can Tachyon deal Sonic damage to Progeny?
2. Is a reshuffle required and if so how many times? (this seems to go against what you would do in a normal game though)
3. Can Tachyon play less than 10 cards in a single turn?
4. Can Tachyon still be standing at the end? This one I doubt because of the story, but it may not require an incap.
Failed Unlock.
Attempted to have 2 Research Grants in play by the time of incap.

Well, I failed, but this game's a doozy...
Met the pre-req games.
Defeated progeny with the base Freedom Five.
Tachyon never went down.
Tachyon dealt no sonic damage to progeny (but some to herself).
Tachyon spent an entire turn at the end with hud goggles and PtL in play from the beginning of the turn playing lightning reflexes into fleet of feet into quick insight in order to reshuffle her deck and play the same cards for as long as I physically could.
Lost track of how many reshuffles that was and how many card plays that was on that turn alone. Reshuffled once before that turn.
No Unlock.
This game was... probably my favorite actually. By all means, here's the data.
Judging by this, assuming we're on the right track, you may be right about Tachyon needing to fall. An alternative is that there's a limit to the amount of sonic damage she can deal herself.
Has anyone tested if Tachyon is allowed to hit Progeny with Hypersonic Assault while the environment has changed it to Melee damage?

I satisfied conditions 1, 2, 3, 4, and 5 from your list, but I'm pretty sure I didn't ever play 10 cards in one turn (based on a hacky Python script, I think I maxed out at 8 plays in a single turn: f5 tachyon fail analysis - Pastebin.com)
I know I don't have logs, folks, but I still have potentially valuable information.
I only shuffled the trash into deck once in my unlock.
Did essentially the exact same thing as my doozy of a game. This time, though, I killed Tachyon. Got the unlock. Here's the log.
I think we've got this, we just need to determine the exact number of cards played.
Judging by my games alone, though, Tachyon being decapped is a near certainty.
Is dealing Sonic damage ok? Do you need to shuffle the deck?
I’m guessing “play 10 cards in one turn” and “get killed, then win” are conditions. But the others are up in the air. I’ll try doing it with Sonic damage, and not intentionally TRYING to shuffle, but not avoiding it either.
Just got the unlock with one shuffle, no Hypersonic Assault, an empty deck, and a maximum-play turn. I’ll post the log Monday after I get a chance to clean up the undo’s.
Just got the unlock, and I can confirm that it's okay to hit Progeny with Hypersonic Assault, as long as the damage it deals is switched to Melee instead of Sonic.
Ok. You CAN deal Sonic damage. You also don’t need to shuffle twice. I did once, but who knows if you need to. I also died with Burst cards in my trash, and some have not, so that’s not important.
I’d guess it’s as follows:
Do the prerequisites.
Then win against Progeny at Megalopolis with Vanilla FF.
Tachyon must play at least 10 cards in one turn, and must be incapacitated.
Judging by the fact every unlock so far had the trash shuffle at least once, I’d revise that to be:
I’d say it’s one of those two.
That being said, it could be that the shuffling is a side effect of waiting until you can pull off the 10-card combo. (Also, 3 if the cards in the combo MAKE you draw cards.) So whether it’s required, or an unintended side effect, is in need of further testing.
I had a fail (log above–still need to remove the undo’s) in which I had Tachyon performed the mega-turn and was incapped but did not get the unlock. If the reason wasn’t the sonic damage, it might be the lack of reshuffling.
